For peripheral neural infusion of LA using 6-12mls/hr of bupivacaine 0.125%, levobupivacaine or 0.2% ropivacaine, would accidental infusion directly into the vein result in LA toxicity in a 6okg adult?

depends on the individual. i believe there is wide inter-patient variability in sensitivity to LA toxicity. intravenous infusion at any rate could cause problems in a patient with the right co-morbidities (ischemic heart disease, conduction abnormalities, metabolic derangements, etc). i'm not convinced that in these patients using a chiral prep provides any added measure of safety.
