40 year old male presented in A&E with multiple overdose. His GCS was 12/15 on arrival and declining. He was suspected of taking large dose of paracetamol, amitryptyline, codeine, tramadol and alcohol. Shortly his GCS dropped to 8/15. However his vital signs were stable and no significant changes were seen on ECG. We decided to use Lipid Rescue in an attempt to avoid intubation. Patient received total of 500 mls of 20% Intralipid. His GCS did not improved, nor deteriorate. At this point we received blood results showing very high paracetamol levels requiring treatment. Alcohol level was>2000mg/l. Family revealed there was over 2 g of amitryptyline missing. Patient was then intubated and ventilated. He required overnight admission to ITU. After 12 hours he was extubated and made quickly a full recovery.
