Yesterday, I reversed adverse drug reactions in a 27 y.o. male with altered mental status, hallucinations, aggressive behavior (requiring restraints), sinus tachycardia, elevated blood pressures and periodic clonus posturing. I had only 60 mL of 20% lipid emulsions but within 5-10 minutes, vitals signs stabilized and aggressive behavior resolved; and patient returned to baseline affect. Urine tox screen with enzyme immunoassay indicated only THC > 50 nanogram. Clinical presentations and discussions with poison control suggested a possible “synthetic marijuana” ingestion. My closest advanced care hospital is over 3 hours away by ground and the LipidRescue protocol likely prevented an adverse turn of events and/or medical airlift. It may be very useful for the litany of annual ER visits related to ingestions of "synthetic marijuana" and/or “bath salts”. I was able to discharge the patient this a.m. after <24 hrs. hospitalization and required no additional medication and/or procedures. That's great Dr. Holsworth insofar as this might be the first reversal of synthetic marijuana using lipid emulsion. Let us know what you find when the tox screen comes back so that we can all learn from your experience. Thanks for posting this interesting and potentially very important case. G
