What does "Local Anaesthetic Toxity" mean? Does it mean all anaesthetic including Lydocaine or only Bupivacain?

Do you have any idea?

Hi Vladimir,

I think it is mistake to formulate "Local Anaesthetic" because Lydocaine is not lipofilic as Bupivacaine or Ropivacaine. So I think it is not good idea to use Lipid Emulsions for treatment of overdose of Lidocaine. Moreover it may dangerose for patient.

New guidelines [Neal J., Bernards C., Butterworth J.F.IV, Di Gregorio G., Drasner K., Hejtmanek M.R., Mulroy M.F., Rosenquist R.W., Weinberg G.L. ASRA practice advisory on local anesthetic systemic toxicity. Reg. Anesth. Pain Med.2010. – Vol. 35, №2. – P.152-161] mean ANY local anesthetic.

IMHO, lidocaine is lipophylic enough to enter cell membranes ;)

I agree with Volodymyr. Liodocaine has logP of 2.3 which is highly lipophilic. I am also aware of at least one very dramatic save from pure lidocaine toxicity. The paper by Zausig has significant flaws which should have precluded their unfortunate choice of wording in the title that has led many to believe that lipid only works for bupivacaine overdose but us ineffective for clinical treatment of ropivacaine or mepivacaine (and presumably lidocaine) overdose...this is entirely incorrect.
