What is the contraindication for lipid rescue and acute myocardial infarction? Is there any literature to support this statement.

let me clarify. i think lipid may not be effective when the oxygen and pH are very low. therefore i doubt it would be very helpful when the patient is asphyxiated as in out of hospital cardiac arrest. However, there is now very good evidence in a variety of animal models that lipid emulsion infusion may be beneficial in reducing ischemia reperfusion injury. therefore it may be beneficial to give lipid in the setting of acute MI BUT only if the patient is otherwise metabolically stable (good perfusion, oxygen saturation and pH). timing may be important here, as lipid may be more beneficial after a 10-20 min period of re-perfusion as opposed to giving it during ischemia where tissue pH and oxygen tension is low.
