All very good questions. For the pediatric question i point you to my answer to Fiona's post (adjacent to yours). For the other scenarios we have no clear cut answer. however, for both the morbidly obese and pregnant patient, i would think it reasonable to use the patients' estimated lean body mass, as opposed to their actual weight. however, there are no clear cut, definitive answers to your question. guy
