Is Intralipid indicated for cocaine induced arrhythmia? If the arrhythmia was caused by a therapeutic dose of cocaine, is there any place for Intralipid?
In our case, we did not use it, as cocaine is not an amide LA and the effects of Intralipid in this case were not known to us.

Great question Sylva. The short answer is, "we don't know". The reason it might not work is not related to cocaine being an ester. It's because it's not so lipophilic as bupivacaine. however, it does inhibit the same metabolic steps in fatty acid metabolism as does bupivacaine and one could argue that it might work. We did some preliminary studies a while back that showed lipid pretreatment does reduce cocaaine toxicity. However, further studies are needed. Meantime, I can't imagine that it isn't worth a try....assuming that the patient is well oxygenated.
