According to the Case Management Guideline Intralipid use, the procedure for a 70 kg patient, involves the administration of a 800 ml (in case tha patient receives the repeat bolus). However, the procedure mentiones that the maximum recommended total dose should be 8 ml/kg (that makes, 560 mL). May you clarify this point for me?

Dear friend,

you are absolutly right! The maximum daily dose of lipids is 3g/kg BW. For patient 70 kg BW it would be 210 g of lipids (1000 ml of 20% Lipid Emulsion). This quantity should be infused for 17 hours (max rate of infusion according to lable is 60 ml per hour). If you used 20% Lipid Emulsion with rate of infusion exceed of lable you would get of overdose of lipids. I this case you may kill the patient.Anyway you will take the responsibility for yourself. The producer of Lipid Emulsion is not carry any responsibility for this because they recommend to use their product according to lable.

Four points:
1. if the patient is in cardiac arrest, an adverse effect of rapid lipid administration is not an issue. they are already DEAD.
2. the average reported dose of lipid given for lipid rescue is ~4mL/kg. you do not need to give the upper limit.
3. the LD50 in rats for rapid administration of 20% lipid is 67mL/kg.
4. there have been no reported adverse effects of lipid rescue
