Where is all the info in regards to a lidocaine overdose and treatment with intralipids? Are lipid also helpful or recommended for lidocaine toxicity.

Hi,

I think it is mistake to formulate "Local Anaesthetic" because Lydocaine as not good lipophilic as Bupivacaine or Ropivacaine. So I think it is not good idea to use Lipid Emulsions for treatment of overdose of Lidocaine. Moreover it may be dangerous for patient to infuse Intralipid with extremely high dose. lable of Intralipid is not contain indication to use it for the overdose of any LA.On other hand lable of Lidocaine also is not contain information about using Lipid Emulsion for its overdose. As we are doctors we must keep to the official lable.

There are unquestionably successful cases of lipid reversal of mepivacaine toxicity and more recently an impressive case of reversal of lidocaine toxicity waiting to be published. If your patient is dying oflidocaine overdose, you should use lipid. There is little downside. The 'evidence' against using it is from an isolated heart model of LA toxicity. This should not dictate or limit your clinical management of a patient in extremis. My opinion: USE LIPID for LIDOCAINE TOXICITY!!!!!
