61-year old (ASA Status I) patient with radiusfracture receiving axillary plexusblockade. Block performed with stimulation regulated down to 0,4mA with repeated negative aspiration; at the end of injection (Prilocain 300mg and Ropivacain 100mg) tonic clonic seizure. Termination of convulsion with Midazolam and Propofol. ClinOleic 20% 150ml were applicated, no progression to hemodynamic or rhythmic disorder. Patient awakes after 20min uneventfully without apparent effect of regional anaesthesia. Surgery was thereupon performed under general anaesthesia. Full neurological recovery, no retrospection of the incidence. Lipidinfusion possibly prevented progression to cardiac complications.
