33M received interscalene block with 40ml of ropi 0.75% as single shot (pt refused catheter technique) for shoulder arthroscopy. Had history of significant motion sickness. Block performed without complication under U/S guidance. Pt developed dizziness and significant refractory n/v afterward. Sedation used during block was reversed with narcan. Pt received fluids, zofran x 2, small amount of dexamethasone, droperidol, phenergan all without effect. Hypothesized that significant systemic LA absorption had probably occured that could account for symptoms. Despite continued n/v, case proceeded and he was anesthetized with propofol infusion/LMA for procedure. 250ml of intralipid was infused during the case. Pt awoke without any nausea and was d/c shortly after.
