Post Your Cases > Seizure immediately after placement of femoral nerve block

17 yo female scheduled for a left ACL repair under general anesthesia and femoral nerve block. No significant medical history. Nerve stimulator used for the block. Total of 30 ml of ropivacaine injected in divided doses of 5ml after negative aspiration for heme each time. Two minutes after block completed, the patient went into tonic clonic seizure. Airway was immediately secured. Vitals stable throughout. Intralipid therapy was initiated for emperic prevention of cardiac toxicity. Pt was aware and following all commands thirty minutes after the initial seizure episode. EKG showed new PAC & PVC. Patient was admitted overnight for observation and was discharged home the next day without further sequele. Ropivacaine serum blood sample was sent.

The patient returned a month later for the left ACL repair under general anesthesia and had agreed to get the femoral block again. This time the block was done with ultrasound guidance, which showed another artery very close to the femoral artery. Patient tolerated the block well without complications.
