15 yo 55 kg AAM for hip surgery. Lumbar plexus single injection nerve block with 20 ml 0.5% ropivacaine. Sedated with 2 mg midazolam and 100 mcg fentanyl (slowly sedated. Pt talked to us throughout procedure.) Atraumatic. Single pass. Femoral nerve stimualted with quadricep response lost at 0.44 mA. Negative aspiration every 2-3ml. Pt talking after procedure. About to bring pt to OR and full tonic clonic seizure 5-10 secs. Benzos and intralipid called for. Seizure was not long enough to obtain anything before it stopped (no history of seizures). Oxygen applied. BP stable (SPB 140-160). HR stable (90-110s). Pt breathing without assist. Pt followed commands. Moved all 4 extremities. Oriented x3. No complaints of ringing in ears, metallic taste or perioral numbness. Spoke with mom and surgeon. Proceeded with case. Patient admitted for 23 hour observation.
