Pt is 33 y/o woman. Recieved diltiazem 180mg @145am and 1004am. Amlodipine @ 1004am. Normal dose was Diltiazem 180mg QD.
I was called to the CCU for intubation. Hr was 37. Pt recieved atropine 0.5mg @1151 and 1mg at 1225. By 1635 HR was 36 and BP was 84/52, sat 92% with bag mask assist by ICU RN.
After 50mg ephedrine was given HR was 39 and BP 82/52.
Induction was 4mg Versed and 50mg Rocuronium.
LAST protocol was initiated. AFter the first bolus, HR was 55-57.

Thanks for the post, Lawrence. I trust the patient had a good outcome. As you know, lipid rescue is being used more frequently for a variety of non-local anesthetic drug toxidromes, including CCBs. I would be interested to see a plot of key hemodynamic metrics over time, noting when various interventions, including lipid infusion were made. Thanks again.
