16 yo female presents via EMS with intentional drug overdose of mother's buspirone HCl, 7.5mg. Patient took 12-16 tablets orally, becoming lethargic and decreased responsiveness. Patient transferred via EMS, GC = 13 but stable vital signs. Intralipids 20%, 100mL IV push over 90 seconds. Patient responded in 15 minutes with increased alertness GC = 15 and responded to questions.
Patient remained stable and discharged to home after appropriate psych evaluation/counseling.
No other CNS or respiratory depression observed.

Thanks, Dr. Holsworth for this contribution to the developing experience for use of ILE in settings of pharmaco-tocicity presenting primarily with altered mental status. The predicted logP is 3.43 which would suggest it might partition into a lipid bulk phase, and that lipid rescue therapy could be effective. Can you confirm that aside from the 2 point change in GCS that from the 'foot of the bed' you saw a real clinical improvement? Thanks again.
