LAST following Right Ultrasound Guided Interscalene block. Pt with runs of Vtach within one minute of 30ml of 0.5% bupivacaine with EPI and 8 mg decadron, following negative aspiration q 5 mls. Intralipid protocol initiated withing 2 minutes of recognition, ACLS protocol followed, patient survived, to ICU for a few days and returned home.
