6m old, 8kg, ASA1. Scaphocephaly. 7mg/kg xylocaine with adrenaline infiltration to planned bicoronal scalp incision. 20 mins later: profound, worsening hypotension/drop in ETCO2/SpO2. Extreme ST elevation. Two doses of adrenaline (1mcg/kg). 20% intralipid 2ml/kg fast bolus led to instant resolution of physiology. Follow up 12ml/kg over 50mins infused. No further complications. 12-lead ECG: normal. Echo: incidental PFO, no other structural abnormality. Child woken without incident.
